区域性市级医院院前急救流行病学分析

Pre-hospital First Aid of Epidemiologic Investigation in Jianyang City

目的:弄清县市120院前急救的基本情况,为政府及医院提供政策依据。方法:收集、统计、分析本院2007年1月1日至12月31日的院前急救资料。结果:全年出诊3401次,救治患者3019人次,死亡141人;无效出诊619次,占出诊比例的18.2%;欠费755次,占出诊比例的22.2%;呼救比较集中在上午8时后至晚上24时前,中位数16.471;急救半径中位数9.55km;急救反应时间中位数是18.24m in;院前主要疾病依次是创伤、心血管疾病、脑血管疾病、中毒、小儿内科疾病、神经系统疾病、消化系统疾病、呼吸系统疾病、妇产科疾病。院前死亡前五位的病因依次是心脑血管疾病、创伤、呼吸系统疾病、中毒及溺水;儿科出诊以新生儿、婴儿及学龄儿童为主。结论:创伤是第一位的出诊原因,心脑血管疾病是第一位的死亡原因,新生儿疾病是小儿内科第一位的出诊原因。县市级医院的急救培训应以创伤急救、心肺脑复苏、心脑血管急救为重点。

Objective： To understand the basic state of pre- hospital first aid in native 120 and to provide the policy for county and leaders of hospital. Method： Statistic study of the pre - hospital first aid medical history by daoctor and the emergency note by nurse from January 1 2007 to December 31 2007 was analyzed. To use EXCEL electronic table to record every emergency disease. Result： The emergency disease of total year was 3401. 3019 persons saved. 141persons were dead . 619 cases were inefficiency ,Its percentage was 18.2%, 755 cases were not paid. And it was 22.2%. 120 response time was concentrated from after 8 ： 00am to before 24：00pro. The 9.55 km was first aid radius median and 18.24min was its response time. The squence of emergency disease was trauma, cardiovascular disease, cerebrovascular disease,intoxication, child physical disease, nervous system disease, digestive system disease, respiratory system disease, obstetric and gynecologic disease. The first five major death etiopathogenesis were cardio - cerebrovascular disease, trauma , respiratory system disease , intoxication and drowned. Newborn infant and preschooler were major emergency cases. Conclusion ： The trauma is the first reason for emergency call. cardio - cerebrovascular disease is the first death reason. And newborn disease is the first child physical emergency call. We should short response time and change in - hospital and pre - hospital first aid medical model. At the same time, Our emergency education should make them very important, such as trauma first aid, cardiopulmonary resuscitation and cardio - cerebrovascular first aid.
