肾炎康复片与坎地沙坦酯治疗糖尿病肾病的对照研究

Comparative study of therapeutic effects of Shenyan Kangfu Tablet and candesartan cilexetil on diabetic nephropathy

目的：探讨肾炎康复片（SYKFT）对微量白蛋白尿期糖尿病肾病的疗效。方法：观察微量白蛋白尿期糖尿病肾病（DN）患者90例,随机分为3组：对照（C）组24例给予单纯降糖治疗;坎地沙坦酯（CC）组28例在降糖治疗同时,每日晨服坎地沙坦酯8 mg;肾炎康复片（SYKFT）组26例在降糖治疗的同时,给予肾炎康复片5片口服,3次/d。3组均连续治疗12 w,比较治疗前、治疗后4、8和12 w时组间及组内尿白蛋白排泄率（UAER）等的变化。结果：C组UAER在治疗前后差异无统计学意义（P〉0.05）;CC组和SYKFT组治疗前后差异有统计学意义（P〈0.01~0.05）,治疗12 w后,CC组UAER下降30.7%,SYKFT组下降32.3%;治疗后,CC、SYKFT两组UAER与C组比较差异有统计学意义（P〈0.01~0.05）,但CC、SYKFT两组间第8 w、第12 w治疗后UAER下降差异无统计学意义（P〉0.05）。结论：肾炎康复片能够减少微量白蛋白尿期糖尿病肾病患者的UAER;发生效果的时间较坎地沙坦酯快,12 w时与坎地沙坦酯的疗效相当。

Objective： To investigate the therapeutic effects of Shenyan Kangfu Tablet （SYKFT） on diabetic nephropathy patients with microalbuminuria. Methods ： A total of 90 diabetic nephropathy patients with microalbuminuria were randomly divided into three groups. Patients in control group （n =24） only received glucose - lowering treatment while those in candesartan cilexetil（CC） group （n =28 ） and SYKFT group （n = 26） were given candesartan cilexetil （ 8 mg qd, po） and SYKFT（5 tables tid, po）, respectively, besides glucose - lowering treatment. All patients of three groups were treated for 12 weeks. The levels of urinary albumin excretion rate （UAER）, urea nitrogen, ereatinine, and other indexes before treatment and after treatment for 4,8, 12 weeks were subjected to intergroup and intragroup comparison. Results： UAER levels decreased significantly in both SYKFT group and CC group （P 〈0.01 -0.05 ） after treatment compared with those before treatment, but it did not occur in control group （P 〉 0.05 ）. After 12 - weeks treatment, UAER level reduced by 30.6% and 32.3% in CC group and SYKFT group, respectively. Significant differences of UAER level after treatment were found betWeen CC/SYKFT group and control group （ P 〈 0.01 - 0.05 ）. However, UAER level after treatment for 4 and 8 weeks had no significant differences between CC group and SYKFT group （ P 〉0.05 ）. Conclusion ：SYKFT can decrease UAER level in diabetic nephropathy patients with microalbuminuria more quickly than candesartan cilexetil. It has similar efficacy to candesartan cilexetil after 12 - weeks treatment.
