原发性中枢神经系统淋巴瘤16例临床分析

Clinical analysis of 16 cases with primary central nervous system lymphoma

目的探讨原发性中枢神经系统淋巴瘤的发病机制、临床表现、诊断、治疗及预后。方法回顾性分析16例经病理证实为原发性中枢神经系统淋巴瘤患者的临床特征、影像学特点、治疗方法及预后。结果术前早期诊断2例。术后随访15例,死亡13例,2例存活。术后行放、化疗者中位生存期13.7个月。结论原发性中枢神经系统淋巴瘤术前难以确诊、预后差。手术应以保留患者重要神经功能为前提,强调手术与放、化疗结合的综合治疗。

Objective To explore the mechanism, clinical manifestation, diagnosis, treatment and prognosis of prmiary central nervous system lymphoma （PCNSL）. Methods The clinical data of 16 cases of PCNSL confirmed by pathological examination were analyzed retrospectively, including clinical characteristics, radiological features, treatment methods and prognosis. Results 2 cases were diagnosed preoperatively. 15 cases were treated with radiotherapy and chemotherapy and they were followed-up posteroperation. 13 cases died and 2 cases survived. The median survival time was 13.7 months. Conclusion PCNSL is difficult to be diagnosed preoperation, and prognosis of which is worse. Protecting important nervous function is major purpose in surgical excision. We emphasized treatment of surgical excision combined radiotherapy and chemotherapy.
