瑞芬太尼复合丙泊酚麻醉用于小儿扁桃体切除术的临床观察 被引量:2

摘要 目的探讨瑞芬太尼复合丙泊酚全凭静脉麻醉用于小儿扁桃体及腺样体切除术的麻醉可行性。方法选择慢性扁桃体炎、腺样体肥大患儿40例,ASAⅠ~Ⅱ级。在全麻下行双侧扁桃体及腺样体切除术,所有患儿术前30min肌肉注射苯巴比妥钠5mg/kg,,阿托品0.02mg/kg。以丙泊酚2~2.5mg/kg、维库溴铵0.1mg/kg、瑞芬太尼1μg/kg静脉滴注诱导,气管插管后行机械通气,潮气量8~10ml/kg,术中以丙泊酚4mg/(kg.h)、瑞芬太尼0.2μg/(kg.min)静脉泵注维持麻醉,术中根据麻醉深浅调整输注速度。术中连续监测并分别记录:麻醉前、诱导时、气管插管时、手术开始时、手术开始后20min、拔管时患者的血压(BP),心率(HR)。记录患儿瑞芬太尼、丙泊酚的用量,以及停药后患儿的呼吸,意识恢复时间,术终至拔管时间,术后2d随访观察恶心、呕吐的发生情况。结果所有患儿麻醉过程平稳,BP稍有波动,但各时点的BP变化差异无统计学意义;HR变化明显,各时点HR变化与麻醉前比较明显减慢(P<0.05或P<0.01)瑞芬太尼的用量为(0.23±0.05)μg/(kg.min),丙泊酚的用量为(6.8±1.1)mg/h,患儿在停药后(6.3±4.1)min呼吸恢复,停药后(10.2±5.7)min意识恢复,术终拔管时间为(10.4±4.3)min,术后均能合作。术后有2例患儿有轻度呕吐,其余患儿均无呕吐,恶心、呕吐发生率低。结论瑞芬太尼复合丙泊酚全凭静脉麻醉是小儿扁桃体及腺样体切除手术较好的麻醉选择。
