组织追踪法评价肥厚型心肌病左室收缩协调性和纵向运动功能

Assessment of systolic synchrony and longitudinal movement of left ventricle in hypertrophic cardiomyopathy using tissue tracking

摘要 目的探讨应用组织追踪法(TT)评价肥厚型心肌病(HCM)患者左室收缩协调性和纵向运动功能。方法利用TT技术获取22例HCM患者和20例正常对照者的左室壁运动曲线,分别测量心尖长轴、心尖二腔和心尖四腔切面的二尖瓣环、基底段中部、中段乳头肌水平的收缩期峰值位移(Ds)、相对室壁达峰时间差(△Dt)和峰值位移差(△Ds)。结果与对照组比较,HCM组△Dt明显延长,△Ds明显增大,Ds明显降低,差异有显著性意义。结论HCM患者存在左室收缩协调障碍和纵向运动功能损害,TT能定量评价HCM患者左室收缩协调性和纵向运动功能。 Objective To assess left ventricular systolic synchrony and longitudinal movement function in hypertrophic cardiomyopathy(HCM)using tissue tracking technology(TT). Methotis Twenty-two patients with HCM and 20 healthy subjects as control group were studied at apcial long-axis view, apcial two-chamber view and four-chamber view using TT. Peak displacements (Ds), time to peak difference(△Dt), peak displacement difference(△Ds) were measured in the mitral annular, basal, middle segment of the left ventricle in systolic period respectively. Results △Dt, △Ds were significiently longer and Ds was significiently lower at each level in the HCM patients than those in the healthy subjects. Conclusions The left ventricular systolic synchrony and segmental movement functions along long-axis were impaired in patients with HCM. TT can assess systolic synchrony and longitudinal movement function of the left ventricle in patients with HCM.
