颊黏膜炎性肌纤维母细胞瘤1例报告及文献回顾

Inflammatory myofibroblastic tumor of the buccal mucous:case report and review of the literature

目的探讨口腔颌面部炎性肌纤维母细胞瘤(IMT)的病因、临床病理特点及诊断和治疗的方法。方法报告1例颊黏膜炎性肌纤维母细胞瘤,并对其进行常规病理、免疫组化分析及相关文献回顾。结果患者女性,54岁,临床表现为颊黏膜溃疡性肿物,缓慢生长。光镜下见肿瘤由梭形细胞和大量炎性细胞组成。免疫组化显示Vimentin、SMA阳性,S-100、CD68、CK阴性。术后随访10个月无复发。结论IMT是一种新近命名的少见软组织良性肿瘤,临床上主要依靠肿瘤病理组织学特征及免疫组化检测确诊。口腔颌面部的IMT一般可以通过外科手术切除而彻底治愈,IMT对皮质激素敏感,可与手术联合运用。

Objective To discuss the etiology, clinical and pathologic characteristics, diagnosis, therapy, and prognosis of the inflammatory myofibroblastic tumor（IMT） of oral and maxillofacial region. Methods A case of IMT located in the buccal mucosa was reported. The clinical manifestation , histological feature, diagnosis and treatment methods of IMT were discussed, and relevant literatures of IMT were reviewed. Results The case was female, 54 year old, the tumor presented as ulcer mass with slow growth. Histologically, the tumor was composed of spindle cells and large number of chronic inflammatory cells. Inmmnohistochemical stains demonstrated Vimentin, SMA positivity meanwhile S- 100, CD68, CK negativity. A follow - up for 10 months was done and no any recurrence was found. Conclusion IMT is a rare type of benigin soft tissue tumor. The diagnosis of IMT can be made on the basis of histopathology and immunohistochemistry. No malignant transformation of oral and maxillofacial region IMT has been reported. Surgical eradication is the most valuable method to cure the tumor, and eorticosteroid is recommended to cooperate with surgical treatment.
