彩超在体外碎石治疗输尿管中下段结石中的定位价值

目的进一步探讨彩色多普勒超声在体外碎石治疗输尿管中下段结石中的定位价值。方法先采取仰卧位对输尿管中下段各狭窄部位结石进行细致扫查,定位时采取俯卧位,探头对向患侧,与水平呈50°～70°,选择髂总、髂外动脉、精囊(或子宫动脉)、充盈的膀胱作为标志物定位。对于肥胖患者需空腹和排空大便,必要时可口服番泻叶清洁肠道。结果385例中,输尿管第二狭窄部(跨越髂血管分叉处)结石65例;输尿管第三狭窄部(输尿管与男性输精管或女性阔韧带交叉处)结石82例;第四狭窄部(输尿管进入膀胱壁外缘)结石103例;第五狭窄部(壁内段)结石135例。经上述方法,定位成功率为97.9%。结论在无辅助措施条件下,借助髂总、髂外动脉、精囊(或子宫动脉)和充盈的膀胱作为标志物,应用彩色多普勒超声进行定位,可进一步提高体外碎石的准确性,为临床治疗提供更有价值的信息。
