小梁切除术联合丝裂霉素C及羊膜植入治疗开角型青光眼的远期效果观察

Long-term effect of trabeculectomy with mitomycin C and amniotic membrane implant on patients with primary open-angle glaucoma

目的观察小梁切除术联合丝裂霉素C（MMC）及羊膜植入治疗开角型青光眼的临床效果。方法对24例（32只眼）26～55岁开角型青光眼患者进行小梁切除术联合MMC及羊膜植入治疗。术后随访6—18个月，观察患者视力、眼压及滤过泡情况。结果视力较术前无明显变化。患者术前平均眼压（38．5±7．2）mmHg，与术后18个月平均眼压（16．8±3．9）mmHg相比，差异有统计学意义（P〈0．01）。随访期间有4只眼需药物治疗，11例有功能性的滤过泡。结论小粱切除术联合MMC及羊膜植入能有效降低眼压，效果良好。

Objective To evaluate the long-term effect of trabeculectomy with mitomycin C（MMC） and amniotic membrane（AM） implant in patients with primary open-angle glaucoma. Methods Trabeculectomy with MMC and AM implant was performed on 32 eyes of 24 patients with primary open-angle glaucoma （POAG） aged between 26 - 55 years. Intraocular pressure（IOP） was measured pre and post operatively by Goldmann tonometer. Visual acuity and the bleb appearances were recorded. Follow-up was from 6 to 18 months after surgery. Results Patients IOP of 18 months after operation was significantly decreased compared with that of pre-operation（ P 〈 0.01 ）. At the end of follow-up, IOP was controlled effectively in all patients. Among them,only 4 eyes needed anti-glaucoma medication. There was no significant difference in visual acuity between pre and post operation. Functional filtering blebs were found in 11 eyes. Conclusions NPTS with MMC and AM implant effectively reduce IOP in patients with POAG in long-term follow-up.
