摘要
目的:评价肺内结节性病变CT灌注参数与微血管密度(MVD)的关系和CT灌注成像评价肺内结节性病变的血管生成作用。材料和方法:71例肺结节患者(平均年龄55.5岁,男45例,女26例)行CT灌注成像,造影剂用量40mL。计算血流量(BF,ml/100g/min)、血容量(BV,ml/100g)、平均通过时间(MTT,s)及表面渗透性(PS,ml/100g/min)。取患者的病理标本作免疫组化CD34染色,计数微血管密度(MVD),将CT灌注参数与MVD进行相关性分析。结果:恶性病变的BF,BV,MTT和PS分别为97.30、8.86、6.75、34.52;MVD为59.25。良性病变的灌注参数分别为75.43、12.36、15.35、22.82;MVD为73.46。BF、BV、PS与MVD均成正相关(r分别为0.543、0.375、0.346,P<0.05)。结论:CT灌注参数与MVD相关,是在体评价肿瘤血管生成的理想方法。
Purpose : To correlate the CT perfusion parameters of SPNs with microvessel density (MVD). To determine the validity of CT perfusion in assessing tumor angiogenic activity of pulmonary nodules. Materials and Methords: 71 patients ( mean age 55.5, 26 females) with solitary pulmonary nodules underwent CT peffusiou imaging after intravenous injection of 40 mL of contrast medium. Blood flow ( BF, ml/100g/min) , blood volume (BV, ml/100g) , mean transmit time (MTF, s) and permeability surface area product (PS, ml/100g/min) were analyzed. The CD34 immunohistochemical staining was used for tumor mierovessel counting in 73 patients. CT Perfusion parameters of SPNs were correlatively studied with MVD. Results: The BF, BV, MTT and PS of malignant lesions were 97.30, 8.86, 6.75 and 34.52, respectively. The BF, BV, MTT and PS of benigne lesions were 75.43, 12.36, 15.35 and 22.82 respectively. The MVD of malignant lesions was 59.25, and of benigne lesions was 73.46. There was positive correlation between BF, BV, PS and MVD (r = 0. 543, 0. 375, 0. 346 respectively, P 〈 0.05). Conclusions : CT perfusion parameters showed good correlation with tumor MVD. CT Perfusion is a valuable method for assessing tumor neovascularity of pulmonary nodule lesions.
