329例股骨颈骨折内固定治疗的临床研究

Investigation the femoral neck fracture internal fixation among the 329 cases

目的探讨骨瓣移植在股骨颈骨折空心钉内固定中应用的价值。方法1999年1月至2007年1月，采用空心钉内固定加肌骨瓣治疗股骨颈骨折124例（A组）；采用空心钉内固定加游离骨瓣治疗股骨颈骨折109例（B组）；单纯采用空心钉内固定股骨颈骨折96例（C组）。经过随访，比较3组患者骨折愈合及股骨头坏死情况。结果术后随访12～50个月，无感染、退钉、骨瓣松动等现象。A组有8例股骨头坏死；B组有7例术后股骨头坏死；C组有26例股骨头坏死。结论肌骨瓣移植方式在股骨颈骨折内固定中应用的效果明显，但与游离骨辩移植方式比较并不存在显著差异。

Objective To study the value about the bone graft used in femoral neck fracture cannulated screw internal fixation. Methods Among the 124 cases after cannulated screw internal fixation with Muscle-Bone graft（module A） ; Among the 109 cases with free bone graft（module B） ; Among the 96 cases just used cannulated screw internal fixation（module C）. After the follow-up, we compared the fracture healing and femoral head necrosis among the three. Results Regular follow-up continued 12--50 months. We hadn＇t found the phenomenon about the infection, the loosening about cannulated screw or bone graft. The module A had 8 cases with femoral head necrosis. The module B had 7 cases. The module C had 26 cases. Conclusion Using Muscle-Bone graft has the obvious effect in femoral neck fracture internal fixation,But there are no significant differences in comparison with using free bone graft.
