肠型白塞氏病临床病理特征分析3例

Clinicopathological features of intestinal Behcet's disease:an analysis of 3 cases

目的:探讨肠型白塞氏病的临床特点、病理学特征及鉴别诊断要点.方法:收集肠型白塞氏病确诊患者3例,进行临床资料复习和组织病理学检查.患者均以消化系症状入院,同时患者伴有白塞氏病的主要临床症状.结果:患者溃疡病变多发生于肠系膜附着的对侧,溃疡呈圆形,较深;镜下见肠黏膜组织溃疡形成,周围黏膜下淋巴组织增生显著,主要表现以静脉为主的血管炎症.治疗上,糖皮质激素及免疫抑制剂是基础用药,其预后不令人满意.结论:肠型白塞氏病早期诊断有赖于对病史及临床症状的综合分析.肠型白塞氏病无特异性治疗方法,除非合并严重并发症,一般不主张手术治疗.

AIM： To explore the clinical and pathological features, and differential diagnosis of intestinal Behcet＇s disease （BD）. METHODS： Three cases of intestinal BD collected in the study. Their clinical and were histo- logical findings were examined. The patients met the diagnostic criteria proposed by the international study group of BD. The patients had gastrointestinal symptoms and were accompanied by several main BD features. RESULTS： The intestinal ulcers of BD mostly occurred in the contralateral of mesenteric, round and deep. Histologically, vascular and mucosa around intestinal ulcer were infiltrated by the lymphocytes. Lymphoproliferation was observed, and mainly manifested as vasculitis. Glucocorticoid and immunosuppressant were the dominant prescriptions. The prognosis of intestinal BD remained unsatisfactory.CONCLUSION： Early diagnosis of intestinal BD depends on comprehensive analysis of patients＇ history and clinical symptoms. There is lack of specific treatment for Intestinal BD. General surgery is unnecessary with the exception of severe complications.
