保留脾脏和脾血管的胰体尾切除术

To Retain the Spleen and the Spleen Vascular in the Body and Tail of Pancreas Resection

目的：探讨保留脾脏和脾血管的胰体尾切除术在胰体尾部良性病变或损伤中的应用。方法：回顾我院1996年1月～2009年3月试行保留脾脏及脾血管的胰体尾切除治疗胰体尾部良性病变或损伤的13例患者临床资料，其中胰体尾外伤2例，胰腺浆液性囊腺瘤4例，黏液性囊腺瘤3例，无功能胰岛细胞瘤3例。假性囊肿1例。结果：全组13例患者中11例成功，无腹腔感染，无脾梗死或脾脓肿；术后胰漏2例，经腹腔引流1个月痊愈。术后2周、2个月复查血常规，红细胞、白细胞、血小板正常，血糖正常，IgM、IgG、IgA正常。腹部彩超脾脏外形、大小正常，内部回声均匀。脾脏动静脉血流正常。结论：保留脾脏和脾血管的胰体尾切除术治疗胰体尾部良性病变或损伤是安全可行的，可保留脾脏的抗肿瘤、抗感染的免疫功能。

Objective：To study the application of the body and tail of pancreas resection retaining the spleen and the spleen vascular for the treatment of pancreatic body and tail of benign lesions or injury. Methods： To review thirteen cases of clinical data to be in hospital from March 1996 to March 2009, including two pancreatic body and tail injury, four pancreatic serous cystadenoma, three mucinous cystadenoma, three non - functional islet cell tumor, one pseudocyst. Results： There were eleven cases success, no abdominal infection, no splenic infarction or spleen abscesses; two case of pancreatic leakage, after the peritoneal drainage one month, recovered. Afteroperation two weeks and two month, to review blood and red blood cells and WBC and platelet normal;blood sugar normal; IgM, IgG, IgA normal. Abdominal CDFI spleen shape and size normal, internal echo uniform, spleen arteriovenous normal blood flow. Conclusion： The body and tail of pancreas resection retaining the spleen and the spleen vascular for the treatment of pancreatic body and tail of benign lesions or injury is safe and feasible, the spleen may retain the anti - tumor and anti - infection of immune function.
