综合治疗急性特发性面神经麻痹92例疗效观察

Observation on the comprehensive treatement of 92 cases of acute idiopathic facial palsy

目的观察茎乳孔药物注射加上自我手法按摩和面肌功能锻炼对急性特发性面神经麻痹的临床疗效。方法对确诊为急性特发性面神经麻痹的患者,跟踪随访180例,随机分成2组,进行疗效分析。治疗组92例,在门诊应用2%利多卡因2ml,甲钴胺250μg,ATP针40mg,地塞米松5mg进行茎乳孔注射,1次/d,连续7d;之后隔日1次,共7次;同时加上自我手法按摩和面肌功能锻炼。对照组88例,采用电针、自我手法按摩及面肌功能锻练。治疗前后按House-Brackmann(H-B)面神经分级标准进行评定。结果治疗组的起效时间短于对照组(P<0.01),在Ⅰ级治愈方面也明显优于对照组(P<0.01)。结论采用茎乳孔药物注射治疗急性特发性面神经麻痹疗效确切,开始治疗的时间早,可以缩短治疗进程,经济简便,无后遗症,值得推广应用。

Objective To observe the clinical effect of acute idiopathic facial palsies treated with injecting drugs into stylomastoid foramen combined with massage and facial exercise by self-giving. Methods Total 180 subjects who were acute idiopathic facial palsies and followed up,were derided into two groups,and the effect was analyzed. Treatment group（n= 92）was treated with l idocaine 2ml, mecohalamin 250ug, ATP injection, dexamethasone 5mg, injected into stylomastoid foramen, once a day in seven days,after then every other day one time, seven times in total,and combined with massage and facial exercise by self-giving. Control group（n= 88） was treated with acupuncture, massage and facial exercise by self-giving. Both of them were assessed by the House-Brackmann （H-B） facial nerve grading standards before and after the treatment. Results Treatment group＇s onset time was shorter than control group＇s（P〈0.01） ,and was significantly better than control group in I cured inclass（P〈0.01）. Conclusion The comprehensive treatement is effective, can shorten the treatment process, more economical , no post-effects, and should be applied.
