摘要
目的总结婴幼儿疥疮临床特点,发现误诊的原因,并提出预防误诊的措施。方法回顾分析2004-2008年间46例婴幼儿疥疮误诊临床资料。结果误诊为婴儿湿疹的比例最高,占63%,其余依次为过敏性皮炎17.4%,丘疹性荨麻疹10.9%,脓疱疮6.5%,痱子2.2%。误诊最长时间达4个月。结论未详细询问病史、检查不仔细不全面、乡村医生缺乏皮肤科学知识是误诊主要原因。避免误诊的关键是详细询问病史,采用多种手段做好鉴别诊断。
Objective To summary clinical characteristics of infant scabies and the causes of misdiagnosis, and propose measures to prevent misdiagnosis. Methods Clinical data of 46 cases of infants and young children scabies misdiagnosed from 2004 to 2008 were retrospectively analyzed. Results The proportion of misdiagnosed as infants with eczema was 63% ; the rest were atopic dermatitis of 17.4%, papular urticaria of 10.9%, impetigo of 6.5%, prickly heat of 2.2%. The interval of misdiagnosis was 4 months. Conclusion No asking in detail about the history, didn＇t check carefully and comprehensively, the lack of skin knowledge for rural doctors are the main reasons of misdiagnosis. The key to avoid misdiagnosis are inquiring about detail of the history, doing differential diagnosis by various means.
