局部皮瓣在颜面外伤软组织缺失修复中的应用

Local flap for the reconstruction of facial skin and soft tissue defect

目的 探讨应用局部皮瓣修复面部外伤软组织缺失的美学效果。方法 自2005年7月至2007年12月，收治因各种外伤导致的颜面局部皮肤软组织缺失患者27例，采用鼻唇沟皮瓣、改良的菱形皮瓣、皮下蒂皮瓣、滑行推进皮瓣、A—T皮瓣等多种皮瓣进行修复。结果 27例面部软组织缺失患者，伤口均Ⅰ期愈合，所有皮瓣全部成活，经6～18个月随访，伤口瘢痕不明显，功能满意，美学效果良好。结论 面部外伤导致的皮肤软组织缺失，情况复杂多变，根据软组织缺损的部位、范围、形状，选择适当的局部皮瓣进行修复，能达到理想的美学修复效果。

Objective To explore the aesthetic effect of the repair and reconstruction of facial soft tissue defect by injury with local flap. Methods Different local flaps were designed to repair the facial defects in 27 patients from July 2005 to November 2007. The local flaps included nasolabial sulcus flap, reformative rhomboid flap, subcutaneous pedicle flap, slipping advanced skin flap and A-T flap, eet. Results A total of 27 patients with facial soft tissue defect by injury were treated by local flap transplantation, and all flaps survived well without obvious complications. The repaired tissue was in good match with surrounding tissue. After 6 - 18 months follow-up, all patients gained cosmetic shape and normal function with unconspicuous scars. Conclusion The facial soft tissue defect by injury are complicated. According to the size, shape, location of the defects, different local flaps were designed to recover the defect and ideal aesthetic results could be obtained.
