性腺静脉MSCT血管成像及其临床意义

Angiography and clinical significance of the gonadal veins on multi-slice CT

目的:研究女性性腺静脉MSCT血管成像的显示及其解剖变异,探讨性腺静脉与相关临床因素的关系。方法:155例行下腹及盆腔CT增强扫描的连续女性病例,其中25例普通妇科疾病和40例妇科肿瘤作为病例组,30例无下腹及盆腔疾病作为对照组。在性腺静脉最宽处测量管径,计算两组性腺静脉的显示率,比较两组性腺静脉平均管径和显示率。结果:病例组与对照组的性腺静脉显示率皆大于70%,显示率无显著差别。两组性腺静脉平均管径无统计学意义的差别,但年轻病例组(≤30岁)性腺静脉管径较年长病例组(≥60岁)管径显著细小,有统计学意义的差别。结论:多层螺旋CT血管成像能够较好显示GV走行及其解剖变异。性腺静脉的显示率与生育次数有关,随着年龄的增加而升高。动脉期性腺静脉显影与静脉扩张和功能不全有关。性腺静脉血管束征有助于鉴别卵巢与非卵巢起源的女性盆腔肿块。

Objective：To analyze the visibility of the gonadal vein on multislice CT （MSCT） angiography and to evaluate the clinical correlation factors and its clinical significanee. Methods： 155 female patients were examined by enhanced MSCT successively. Of these 155 patients, 65 with germ cell diseases, including 25 cases with conunon diseases and 40 cases with gynecologieae tumor, were selceted as unhealthy group sunjects. 30 cases free of disease in the abdomen and pelvic were selected as a control group. The maximum diameters of the gonadal veins were measured. Detectability of gonadal veins was calculated. Results： Gonadal veins were shown in more than 70% of the objects in the two group. The detectability of gonadal veins was no significant difference. There was no significant difference in the diameters of the gonadal veins between the control group and study group. The diameters of youthful cases group （ ≤30 years old） were obviously thinner than those of aged cases group （ ≥60 years old）. Ovarian vascular pedicle sign revealed origin of organ with a pelvic mass lesion. Conclusion： Gonadal veins of females can be well depicted by MSCT. Left gonadal veins develop during the arterial phase, which have correlation with gonadal veins diameters. Ovarian vascular pedicle sign can provide useful information for determining the origin of a pelvic masse in females on multi-slice CT.
