摘要 目的探讨18F-脱氧葡萄糖(FDG)PET延迟显像对肺良恶性病变的鉴别诊断价值。方法对36例肺癌患者和20例肺良性病变患者分别进行18F-FDG PET早期头部至盆腔显像和注射后3h胸部延迟显像,测定两次显像病灶的标准摄取值(SUV),并计算18F-FDG的储留指数(RI)。结果肺癌组延迟显像SUV明显高于肺良性病变组(P<0.01),肺癌组的RI也明显高于肺良性病变组(P<0.01);对于早期显像SUV>2.5的肺部结节,以RI>20%为阈值,延迟显像对肺良恶性病变鉴别诊断的灵敏度、特异性和准确性分别为90.0%、80.0%和86.7%。结论18F-FDG PET延迟显像有助于肺良恶性病变的鉴别诊断,结合应用SUV及RI两项指标可提高诊断准确率。 Objective To explore the value of ^18 F-FDG PET delayed imaging in differentiation of malignant from benign lung nodules. Methods Thirty-six patients with malignant lung lesions and 20 patients with benign lesions underwent ^18 F-FDG PET early and 3h delayed imaging after intravenous injection of FDG. The standardized uptake value(SUV) was calculated for both time points, and retention index(RI) was also calculated. Results The SUV and RI of the lesions were significantly higher in lung cancer group than in. benign lung nodules group (P〈0. 01 ). Take SUV〉2.5 combining RI as standard, the sensitivity, speciality and accuracy of delayed imaging in differentiation of malignant from benign lung nodules was 90. 0%, 80. 0% and 86. 7% respectively. Conclusion ^18 F-FDG PET delayed imaging is a reliable imaging protocol for differentiating malignant from benign lung nodules. Delayed imaging combining SUV and RI can improve the accuracy of diagnosis.
