摘要
目的观察参芪扶正注射液联合FOLFOX4方案治疗晚期胃癌的临床疗效及不良反应。方法将48例经病理检查确诊为晚期胃癌的患者随机分为两组。治疗组24例,每日给予参芪扶正注射液250ml,静脉滴注,共10天,联合采用FOLFOX4化疗方案:奥沙利铂85mg/m2,静脉滴注2h,第1天;亚叶酸钙200mg/m2,静脉滴注2h,第1、2天;氟尿嘧啶400mg/m2,静脉推注,第1、2天,氟尿嘧啶600mg/m2,持续静脉泵输注22h,第1、2天。每2周为1个周期。对照组24例,单用FOLFOX4方案(用药与治疗组相同)。治疗4周期后评价疗效及毒副反应。对两组的缓解率、生活质量改善率、不良反应进行分析比较。结果治疗组与对照组的缓解率分别为62.5%(15/24)和50.0%(12/24),有显著性差异(P<0.05);生活质量改善率分别为83.3%(20/24)和50.0%(12/24),有显著性差异(P<0.05);两组主要不良反应白细胞减少、腹泻、口腔炎、神经毒性和脱发等指标的差异具有显著性(P<0.05)。结论参芪扶正注射液联合FOL-FOX4方案治疗晚期胃癌,在疗效、生存质量方面优于单用FOLFOX4方案,且不良反应轻。
Objective To observe the efficacy and toxicities of Shenqi Fuzheng Injection combined with FOLFOX4 regimen in the treatment of patients with advanced gastric carcinoma （AGC）. Methods 48 patients with AGC were randomly divided into two groups. Patients in the treatment group were given FOLFOX4 regimen chemotherapy and Shenqi Fuzheng Injection 250 ml introvenously once a day for 10 days. Patients in the control group were given FOLFOX4 chemotherapy only. The FOLFOX4 chemotherapy included L-OHP 85 mg/m^2 in dl, CF 200 mg/m^2 in dl and d2, followed by 5-FU 400 mg/m^2 in dl and d2 by bolus injection and 5-FU 600 mg/m^2 by continuous injection for 22 h from dl to d2. The treatment was repearted every 14 day. The response rate, QOL and toxicities were analyzed after four courses of treatment. Results The response rate （RR） of the two groups was 62.5% （ 15/24 ） and 50.0% （ 12/24）, respectively. There was significant difference （P 〈 0.05）. The improvement of QOL was noted in 83.3% （20/24） patients in the treatment group and 50.0% （12/24） in the control group, respectively. There was significant difference （ P 〈 0.05 ）. The main side effects of the two groups also had significant difference included ： granulocytopenia, diarrhea,stomatitis,neurotoxicity and alopecia. Conclusion Shenqi Fuzheng Injection combined with FOLFOX4 regimen is more effective in the treatment of patients with AGC with less toxicities.
