低场MRI液体衰减反转恢复序列在脑弥漫性轴索损伤中的应用

摘要 目的探讨低场MRI液体衰减反转恢复(fluid attenuated inversion recovery,FLAIR)序列在外伤性弥漫性轴索损伤的应用价值。方法对20例弥漫性轴索损伤患者的临床及影像学资料进行回顾性分析。结果本组病例显示脑白质病灶呈斑点状、小片状,部分伴水肿带。病灶分布:大脑皮质和髓质交界处(17例),胼胝体出血灶(11例),脑干病灶(7例),三脑室周围出血灶(4例),小脑(4例)。部分病例同时具有以上表现。本组随访结果提示中线区域(尤其Adams分级Ⅲ级者)、病灶多发、面积大者及合并其他颅脑损伤者苏醒及预后相对较差。结论FLAIR对弥漫性轴索损伤具有较好的诊断价值,有助于对预后的评价。 Objective To investigate the value and clinical application of low - field intensity MRI with fluid - attenuated inversion - recovery ( FLAIR ) sequency in diffuse axonal injury (DAI). Methods 20 cases with diffuse axonal injury were analyzed retrospectively. Results 122 lesions in suhstantiaalba were displayed with FLAIR and 93 with conventional T2WI, 75 with CT only. The lesions were clearer in FLAIR than those showed in T2WI or CT. Small hemorrhagic loci in cistern can be easily shown in FLAIR and cant be done in conventional T2WI or CT. FLAIR was particular of superiority to small focus and diseases amalgamated. The result hinted that lesions in midline ( Adams Ⅲ especially) or great - area or eombinated with other lesions in brain could lead to bad prognosis. Conclusion FLAIR was useful to DAI and could help estimate prognosis.
