Total pleural decortication were employed in treatment of 20 patients with chronic empyema. In 4 patients of them lobectomy was performed simultaneously, wedge pulmonary resection in one patient and localized thoracoplasty in another patient. There was no operative death and complication. The technique of total pleural decortication was describeds, and the role of the procedure in treatm of chronic empyema and the importance of CT scan in decision of the operation were discussed.
