摘要
目的探讨乳腺三算子及血氧功能成像技术在早期乳腺癌诊断中的应用价值。方法分析48例早期乳腺癌的三算子及血氧特征,结合病理诊断,与临床资料、超声检查及钼靶X线对照。结果(1)乳腺癌以肿块为主者38例,三算子成像呈深灰影;以钙化为主者5例,灰影呈现条片状,为中~浅灰影;肿块伴钙化者6例,灰影范围与肿块大小表现不全一致。48例均有边缘模糊,等灰度曲线局灶异常表现;39例有异常血管表现。(2)血氧检查有高血低氧表现42例,高血氧平3例,血平氧平2例,高血高氧1例。无一例低血高氧。(3)病变敏感性:三算子血氧系统87.5%(42/48),临床检查64.6%(31/48),X线83.7%(36/43),超声83.3%(40/48);三者结合敏感性提高至95.8%(46/48)。结论乳腺三算子及血氧功能成像技术在乳腺癌早期诊断中具有独特的临床价值,与钼靶及超声结合可提高早期乳腺癌的诊断率。
Objective To evaluate the application of three-operators and blood-oxygen functional image (TOBOFI) technology in the diagnosis of early breast cancer. Methods The cheracteristies of 48 cases with early breast cancer on TOBOFI were retrospectively compared with the clinical examination, ultrasonographic (US) and mammographic findings. Results ( 1 ) Thirty-eight cases were mass-based breast cancers,in which the thri-operators imaging showed dark gray shade. Of 38 cases, calcification was the main feature in 5 cases, in which the medium to weak gray shade shaped as strip laminated primarily,mass with calcification was seen in 6 cases with variant features of gray shade and tumor size. The blurry edge and abnormal display of gradation curve were found in 48 cases. An unusual blood vessel performance was seen in 39 cases. (2)Fourty-two patients showed high blood and low oxygen level, 3 patients showed high blood and general oxygen level, 2 patients had general blood and oxygen, 1 patient showed high blood and high oxygen. No cases showed low blood and high oxygen. (3)The sensitivity was 87.5% (42/48) detected by TOBOFI, 64. 6% (31/48) by clinical examination,83.7% (36/43) by mammography, 83.3% (40/48) by US. The sensitivity was improved to 95. 8% (46/48) by combined use of TOBOFI,mammography and US. Conclution The TOBOFI system has the unique clinical value in diagnosing early breast cancer, especially in combination of mammography and US.
