压力支持+叹气混合通气模式在重度创伤后ARDS治疗中的应用 被引量:3

APPLICATION OF A PS + SIGH MIXED MECHANICAL VENTILATION MODE IN THE TREATMENT OF ARDS CAUSED BY SEVERE INJURY

摘要 目的:探讨重度创伤后ARDS机械通气的最佳混合模式。方法:对38例重度创伤后ARDS的临床资料进行回顾性分析,比较压力支持+叹气(PS+Sigh)混合通气模式和呼气未正压(PEEP)模式对重度创伤后ARDS的治疗效果。结果:PS+Sigh混合通气模式和PEEP模式对重度创伤后ARDS治疗前后PaO2、SaO2有显著差异(P<0.01),HB、RR也有差异(P<0.05),PaCO2、MBP无明显差异(P>0.05),而且两种方法的各项监测指标对比无明显差异(P>0.05)。结论:PS+Sigh混合通气模式可以做为重度创伤后ARDS的一种机械通气模式,在危重急症的抢救中优先考虑选用。 To investigate the new methods of mechanical ventilation treatment in patients with acute respiratory distress syn drome (ARDS) caused by severe injury. Methods: We reviewed 38 cases of ARDS caused by severe injury, and compared the effect of treatment between PS+Sigh mixed mode group and traditional PEEP group. Results: The index of multifunction physio logic monitor and blood gas analysis showed that there were remarkable differences between pretreatment and post-treatment in PaO2、SaO2(P<0.01). There were differences in HR、RR (P<0.05), but no difference in PaCO2、MBP (P> 0.05). The results showed that: there were no obvious differences between PS+Sigh mixed mode group and PEEP group (P>0.05). Conclusion: Our data suggest that PS+Sigh mixed mode appear to be a new useful method of mechanical ventilation treatment in patients with ARDS caused by severe injury.
