顽固性鼻出血介入治疗中栓塞剂的选择

The Selection of Embolization Material in Interventional Treatment of Obstinate Epistaxis

目的:探讨顽固性鼻出血介入栓塞材料的选择对治疗效果的影响。材料与方法:对搜集的12例顽固性鼻出血患者行全脑血管造影,确定出血部位及其供血动脉。超选择插管进入出血分支动脉近端,透视下低压缓慢注入栓塞剂,血流缓慢后进行造影复查。3个月后随访鼻出血复发情况。结果:12例顽固性鼻出血患者经介入栓塞治疗后,造影复查异常染色消失,出血停止。随访发现12例顽固性鼻出血患者,其中10例治疗效果良好,2例在介入栓塞后2～3个月内鼻出血复发,出血量及次数有所减少,行射频治疗后未再出血。结论:栓塞剂的选择直接影响鼻出血介入栓塞的治疗效果。

Purpose： to investigate the effect of the selection of the interventional embolization material in the treatment of obstinate epistaxis. Materials and Methods： The 12 patients of obstinate epistacis underment cerebral amgio contrast exami- nation in order to show the bleeding points and the artery of blood supply. After the super selective catheter was inserted into the branches of artery by percutaneous puncture, the embolization material was injected slowly with low pressure. Contrast examination was performed until the speed of blood slowly. Results： After intervenfional treatment was performed in 12 cases of obstihate epistaxis, abnormal stainings were disappeared and bleeding stopped by recontrast examination. The 12 patients were followed up and showed that the effect of treatment alleviated evidently in 10 cases and rebleeding occurred in 2 cases who blood was reduced after 2 to 3 months. Radio frequency therapy was performed on them and no rebleeding happened in follow up. Conclusion ： The selection of embolization material directly affect the affect of interventional embolization treatment in epistaxis.
