护理干预对慢性乙肝患者心理状态及治疗依从性的影响

Impact of nursing intervention on the psychological state and treatment compliance of patients with chronic hepatitis B

目的:探讨护理干预对慢性乙型肝炎患者心理状态及治疗依从性的影响。方法:将200例接受α-干扰素治疗的慢性乙型肝炎患者随机分为干预组105例和对照组95例。对照组采用常规护理,干预组在此基础上运用密切观察、心理干预、对症处理等综合干预方法。采用SCL-90量表比较两组焦虑、抑郁发生情况及患者的治疗完成情况。结果:于治疗前、治疗6、12个月时进行SCL-90评估,干预组焦虑、抑郁发生率显著低于对照组(P<0.05);干预组全程治疗率干预组显著高于对照组(P<0.01)。结论:密切观察、心理干预、对症处理等护理干预措施可降低慢性乙肝患者不良心理反应及接受α-干扰素治疗所致的焦虑、抑郁发生率,改善其心理状态,提高治疗依从性及其治疗效果,提高患者的生活及生存质量。

Objective ： To study the impact of nursing intervention on the psychological state and compliance of patients with chronic hepatitis B. Methods ： 200 patients who received α - interferon treatment of chronic hepatitis B were randomly divided into intervention group （ n = 105 ） and control group （ n = 95 ）. Conventional nursing care was taken in control group and com- prehensive nursing intervention such as close observation, psychological intervention and treatment of complications was applied based on conventional nursing care in intervention group. The SCL - 90 scale was used to compare the occurrence of anxiety and depression of the patients and the completion of the treatment. Results： The incidence of anxiety and depression was significantly lower in intervention group than that in control group according to the assessment by SCL -90 before treatment and when the treatment was given for 6 to 12 months （ P 〈 0.05 ）. The whole course treatment rate in intervention group was significantly higher than that in control group （P 〈0.01 ）. Conclusion： The comprehensive nursing intervention can reduce the adverse psychological reactions and the incidence of anxiety and depression due to α -interferon therapy, improve patient＇s mental state, the treatment compliance, therapeutic effect as well as quality of life of patients in the treatment of patients with chronic hepatitis B.
