空肠腺癌1例及文献复习

Adenocarcinoma of the small intestine:1 case report and review of literature

目的:探讨小肠腺癌的临床表现及病理特点。方法:对1例空肠腺癌进行组织学表现及免疫组化结果观察和文献复习。结果:组织学特点:癌组织腺管状排列,分化比较好,癌细胞胞浆丰富,核大深染,异型性明显,可见病理性核分裂像,部分区域可见黏液组织,肿瘤侵及管壁全层。免疫组化特点:癌细胞呈CEA阳性,CK阳性,CK18阳性及EMA阳性。结论:小肠腺癌是一种少见的恶性肿瘤,临床表现缺乏特异性,需通过钡餐造影和腹部CT等方法协助诊断,手术是其最主要的治疗手段。

Objective ：To investigate the clinical features and pathological characteristics of adenocarcinoma of the small intestine. Methods： One case of adenocarcinoma of the small intestine was reported and the pathological features were studied by microscopy and immunohistochemistry. Results： Histology characteristics ： the cancer cells which have rich cytoplasm and big nucleus grow in gland tubular with good differentiation. Obvious atypia and pathologic mitosis can be observed, and the partial region was mucus. The tumor invaded entire wall of intestine. Immunohistochemistry showed that the tumor cells were positive for CEA, CK, CK18 and EMA. Conelusions：Adenocarcinoma of the small intestine is a rare tumor and because of its nonspecific symptoms,it is usually diagnosed by CT and barium contrast. Complete surgical resection is the main treatment.
