Selection and evaluation of treatment methods for benign pancreatic head mass

胰头部良性肿块大致分为肿瘤性和非肿瘤性两大类。由于病变特点和转归不同,其治疗方法也不尽相同。首先应重视术前的可切除性评估,CT及MRI检查是可切除性评估的重要手段。对于可切除的胰头部良性肿块,应以彻底性切除为最终目标;对于无法切除的肿块,应以解除症状和预防并发症为主。在选择具体术式时,应综合分析,依据病变的局部条件与全身状况、病人及家属对疾病的理解与认识程度、科室专家会诊意见以及术者对拟采用术式的掌控程度。

Pancreatic head benign mass can generally be divided into tumor and non-tumor lesion. There are some differences in treatment methods between two kinds of masses due to their different outcome and prognosis. Preoperative assessment of resectability should be emphasized, and CT and MRI are the important tool for assessment of reseetability. For the resectable benign pancreatic head lesion, the ultimate purpose of treatment should be to resect the mass thoroughly. While for the unresectable mass, the main therapeutic task is to relieve the main symptoms and prevent complications caused by itself. The specific operation procedure should be selected through comprehensive analysis, basing on the local conditions of lesions and patients＇ systemic conditions, the extent of understanding by patients and their relatives about the disease, expert consultation, as well as proficiency of surgeons in the procedure to be adopted.
