紫杉醇单药周疗联合适形放射治疗老年局部晚期非小细胞肺癌的临床分析

目的探讨三维适形放射治疗（3D-CRT）联合单药紫杉醇每周方案同步化疗治疗局部晚期非小细胞肺癌的近期疗效和毒副反应。方法56例Ⅲ期非小细胞肺癌患者采用三维适形放疗，6MVX射线，95％PTV／60Gy／2Gy／30f，同步每周紫杉醇单药静脉化疗，40mg／m^2，连续6周。结果原发病灶近期客观总有效率为82．1％；纵隔转移淋巴结总有效率为91．1％，近期毒副反应主要是骨髓抑制、胃肠道反应和轻中度放射性食管炎、放射性肺炎，患者耐受性满意。结论三维适形放疗与单药紫杉醇每周同步化疗治疗局部晚期非小细胞肺癌有较好近期疗效。

Objective To explore clinical short-term effect and acute side effects of three dimensional conformal radiotherapy （3D-CRT） combined with Paclitaxel regimen chemotherapy in treatment of locally advanced non-small cell lung cancer （NSCLC）. Methods Fifty-six patients of NSCLC Stage Ⅲ were treated with 3D-CRT combined with Paclitaxel regimen chemotherapy every week. Results The overall response rate was 83% for primary tumor, and 90% for metastatic mediastinal lymph nodes. The acute side effects were mostly bone marrow suppression, nausea, vomiting, radiation-induced esophagitis and pneumonitis with good tolerance. Conclusion 3D-CRT combined with Paclitaxel is well tolerated in most patients with locally advanced NSCLC. Its short-term clinical effect is good.
