短期应用诺和锐30特充强化治疗对初发2型糖尿病胰岛功能的影响

摘要 目的研究短期应用诺和锐30特充强化治疗对初发2型糖尿病患者胰岛β细胞功能的影响。方法对47例初发2型糖尿病患者进行诺和锐30特充强化治疗,对治疗前、后不同时期的空腹血糖(FPG)、餐后2h血糖(2hPG)、胰岛素水平(Fins)、空腹C-肽分泌水平(FCP)、餐后2h血浆C-肽(PCP2H)的变化及稳态模型β细胞功能指数(HOMA-B)、稳态模型β细胞的胰岛素抵抗指数(HOMA-IR)进行评估。结果诺和锐30特充强化治疗后,47例患者的FPG、2hPG水平较治疗前显著下降(P<0.01);FCP、PCP2H和HOMA-B较治疗前显著增加(P=0.00);HOMA-IR由治疗前的(0.35±0.21)降低为(0.23±0.19),差异有显著性(P=0.025)。结论短期应用诺和锐30特充强化治疗可显著改善初发2型糖尿病患者胰岛β细胞功能,迅速纠正血糖异常。 [Objective] To study the effects of β-cell function in newly diagnosed type 2 diabetic patients. [Methods] 47 newly diagnosed type 2 diabetic patients were treated with transient intensive insulin aspart 30 infusion for two weeks. Fasting plasma glucose (FPG), 2 hours postprandial glucose (2 h PG), first-phase secretion of insulin,C-peptide,Homa B index and Homa IR index were compared. [Results] Two weeks after the treatment, the FPG and 2 h PG were significantly decreased (P 〈0.01). The C-peptide, PCP 2h first-phase secretion of insulin and the Homa B index were significantly increased (P =0.00), the Homa IR index was sigificantly decreased (P=0.025). [Conclusion] The good glycemic control and improvement of β-cell function can be induced by intensive insulin aspart 30 treatment in newly diagnosed type 2 diabetic patients.
