少见部位骨巨细胞瘤的X线诊断(附20例报告)

The Value of X-ray in the Diagnosis of Giant Cell Tumor of Bone in the Rare Site(Report of 20 Cases)

目的提高X线检查对少见部位骨巨细胞瘤的认识。方法对经手术病理证实的20例少见部位骨巨细胞瘤X线表现进行回顾性分析。20例均行常规X线检查,其中5例行CT检查。结果枕骨斜坡1例,下颌骨3例,跟骨2例,耻骨2例,肋骨4例,脊柱8例。X线主要表现为多房型溶骨性膨胀性破坏13例,单房型溶骨性膨胀性破坏5例,病灶内可见皂泡样改变,无钙化,病灶边缘无硬化,病灶周围骨质可见钻孔样改变6例。结论平片仍是本病诊断的首先方法,解剖结构复杂的部位,需行CT和MRI检查,以提供更多的信息,提高诊断准确率。

Objective To study the value of X-ray in the diagnosis of giant cell tumor（GCT） of bone in the rare site. Methods X-ray findings of 20 cases of GCT in the rare site which was proved by surgery and pathology were analyzed retrospectively. All of the 20 patients underwent X-ray examinations. CT was performed in 5 of them. Results Of the 20 cases ,there was 1 case with tumors at occipi- tal slopes, 3 cases at mandible, 2 at calcaneus,2 at pubic bone ,4 at rib and another 8 at spine . The prominent X-ray images showed multi-room 13 cases and single-room 5 cases osteolytic destruction of vertebral body with definite expansion. There was soap bubble changes inside the tumor, without the calcification and the sclerotic margin. Conclusion X-ray film may still be the first selection for giant cell tumor of bone in the rare site, CT and MRI are of significant value and play an important role in the diagnosis of this disease.
