摘要
目的:观察鞘内泵入布托啡诺对神经病理性疼痛大鼠脊髓P物质的影响,探讨鞘内泵入布托啡诺治疗神经病理性疼痛的可行性。方法:24只SD大鼠随机分为3组,空白对照组(B组),不实施任何手术;对照组(C组),右肢制备坐骨神经慢性压迫损伤模型(CCI)及鞘内置管后鞘内泵入生理盐水10μL/d;布托啡诺组(TB组),CCI及鞘内置管后泵入布托啡诺24μg/d。C组和TB组术后第4天开始鞘内给药,连续给药7d。第11天于给药2h后大鼠灌注固定,采用免疫组化方法检测脊髓背角P物质。结果:成功制作CCI模型,无鞘内导管脱出现象,且注药成功,无疼痛行为学差异。与B组相比,C组和TB组脊髓背角P物质表达明显增加(P<0.01);与C组相比,TB组脊髓背角P物质表达明显降低(P<0.01)。结论:鞘内泵入布托啡诺可以通过减少脊髓P物质表达而发挥镇痛作用,布托啡诺可以通过鞘内给药治疗神经病理性疼痛。
Objective To investigate the effect of intrathecal butorphanol pumping on the expression of spinal substance P in rats with neuropathic pain. Methods 24 Sprague-Dawley rats were randomly divided into 3 groups: 8 in blank control group (group B) received no intervention; 8 in control group (group C) received chronic compressive injuries (CCI) and 4 days later received intrathecal normal saline (10μL/d) pumping in right limbs for 7 days; 8 in butorphanol group (group TB) received CCI and 4 days later received intrathecal butorphanol (24μg/d) pumping in right limbs for 7 days. Expression of substance P in spinal dorsal horn was detected by immunohistochemistry 11 days after CCI. Results The expression of substance P was significantly lower in group B than those in group C and group TB (P 〈 0.01), and was lower in group TB than that in group C (P 〈 0.01). Conclusion Intrathecal butorphanol pumping can be used to treat neuropathic pain according to its effect of reducing the expression of substance P in spinal dorsal horn.
