摘要
目的:应用64排螺旋CT对左心房与肺静脉形态结构的重建,了解分析左心房与肺静脉的具体形态。材料与方法:房颤患者组106例,男63例,女43例,年龄30~70岁。对照组136例,男78例,女58例,年龄22~73岁。所有患者均行64排螺旋CT检查,图像后处理应用3D成像及内镜等技术对肺静脉、左心房等解剖结构进行构建。结果:(1)64排螺旋CT能构建肺静脉形态,可以准确测出肺静脉开口管径的大小、肺静脉的走向及肺静脉的数量;(2)64排螺旋CT能完整构建左心房形态,观察左心房是否有附壁血栓形成。(3)了解肺静脉有三种变异,变异率达9.5%,与性别无明显关系。结论:64排螺旋CT能完整构建肺静脉和左心房的形态结构,为了解左心房和肺静脉开口的结构提供了无创性的信息,在房颤治疗中可选择合适管径的导管进行消融,了解消融术后肺静脉开口处有无狭窄,闭塞或其他异常。
Purpose: Reconstruction to use the feasibility of the demonstration for morphologic characteristics of pulmonary vein (PV)and left atrium (LA)with 64 -multislice computed tomography (MSCT), to comprehend the morphologic of pulmonary vein (PV)and left atrium (LA). Materials and Methods: 106 patients in atrial frillation(AF)group, 63 patients were men, 43 were women, they ages were among 30 - 70years. 136 subjects without history of AF in control group, 78 patients were men, 58 were women, they ages were among 22 - 73years. we studied morphologle characteristics of the pulmonary vein and left atrium in all patients undergone 3D imaging and virtual endoscopy with 64 - slice MSCT scanner. Resuits: ( 1 ) 64 - slice MSCT can accurate reconstruct properly the morphology, number, size of orifice and flow route of pulmonary veins; among them, the sizes of orifice of pulmonary veins in AF group and control group showed no significant difference. (2) The reconstruction images of left atrial appendages (LAA) showed that whether has mural thrombus formed. (3) It is showed vein has three types of pulmonary disease variability, the mutation rare is 9. 5%, and it has nothing to do wih age. Conclusion: Comelete demonstration of pulmonary vein and left atrium through 3D reconstruction of MSCT, and provided the noninvasive informain, optionaled the affable diameer of RFCA for AF with simultaneous, and finded out the narrow, atresia or other variability after simultaneous.
