胰腺外伤及术后并发症的治疗

The treatment of pancreatic trauma and surgical complications

目的提高胰腺外伤及术后并发症的疗效。方法回顾性总结15年来手术治疗胰腺外伤48例。术中对损伤的胰腺组织进行了彻底清创、止血。在充分腹腔引流的同时,对胰腺外伤较重的18例采用胰管内多孔支架管引流,全组病例术后进行腹腔冲洗,并将引流胰液再注入十二指肠。结果胰腺外伤48例,发生胰瘘15例,无其他并发症发生,对胰腺外伤及术后并发症的治疗获得了满意的效果。结论外伤胰腺组织的彻底清创,是预防和治疗胰腺外伤术后并发症成败的关键。

Objective To elevate the therapeutic results of patients suffering from pancreas injury.Method 48 pa- tients with pancreas injury were treated in the past 15 years.Of them 47 cases were complicated with other vis- cera injury.All patients underwent surgical treatment.The damaged pancreas tissue was debrided thoroughly.In eighteen patients with serious pancreas injury,a multiparous stent was placed in pancreas duct to drain pancreas juice.The pancreas juice was collected and sent back into the duodenum.Result Pancreas fistula developed in 15 patients and all healed.Conclusion We observed that the key to the treatment of pancreas injury was to debride the injuried pancreas tissue thoroughly,abdominal cavity drainage and lavage and to prevent possible complications.
