针刀松解联合硬膜外注射治疗腰椎间盘突出症

Clinical study of peridural injection and acupotomy on herniation of lumbar intervertebral disc

目的观察针刀松解联合硬膜外注射治疗腰椎间盘突出症的临床疗效。方法将74例腰椎间盘突出症的患者随机分为治疗组和观察组。其中治疗组38例,对照组36例。治疗组采用针刀松解联合硬膜外注射,对照组采用硬膜外注射治疗。运用改良日本骨科学会下腰痛评分法(M-JOA)进行临床疗效的评估,并比较两种治疗方法的临床效果。结果M-JOA计分结果,两组治疗前后组内比较,差异有统计学意义,2种方法治疗均有效;两组治疗前后差值比较,差异有统计学意义,治疗组优于对照组。两组病例综合疗效优良率比较,P<0.05,差异有统计学意义,治疗组优良率明显高于对照组。结论针刀松解联合硬膜外注射治疗腰椎间盘突出症疗效确切,操作简便、安全,值得推广。

Objective To observe the clinical effect of acupotomy combined with peridural injection on herniation of intervertebral disc. Methods 74 cases were randomly divided into 2 groups,38 cases（treatment group） were treated with acupotomy and peridural injection, while 36 cases（control group） were only treated with peridural injection .According to the improved assessment of low back pain in Japan Orthopedic Association （M-JOA）, to compare the clinical effect of the two methods. Results Comparing the two groups before and after treatment according to the score of M-JOA ,there were statistically significant difference ,and both of the methods are effective; comparing the integrated effect of the curative rate, the difference was statistically significant （P=0.002）,the treatment group was significantly superior to the control group. Conclusion The treatment of the peridural injection and aeupotomy on herniation of intervertebral disc is significantly effective. And the operation is simple and safe as well .It is worthwhile to be widely used.
