C-erbB-2、P^(53)、P^(21)基因过度表达与乳腺癌的预后

摘要 目的研究乳腺癌组织中C-erbB-2、P53、P21基因的过度表达与其预后的关系。材料和方法应用免疫组化方法对90例乳腺癌组织检测其C-erbB-2、P53、P21基因的表达情况。结果C-erbB-2、P53、P21在原发灶中的阳性表达率分别为64.4%,63.3%和35.6%。多基因的阳性表达率为57.8%。C-erbB-2、P53的阳性表达率明显高于P21(P<0.01)。原发灶与转移淋巴结之间的基因过度表达率无显著差异。P53的阳性表达与癌肿大小,浸润程度,淋巴结转移及分期显著正相关。C-erbB-2的阳性表达与淋巴结转移和分期呈正相关。而P21则与上述指标无相关性。结论C-erbB-2、P53的过度表达提示乳腺癌预后差,多基因阳性提示乳腺癌预后差、C-erbB-2、P53、P21多基因过度表达的检测对乳腺癌的预后判断优于其中单个基因过度表达的检测。 ?Purpose:To research the overexpression of C-erbB-2,P53 and P21 in breast cancer andits prognostic significance. Material and Method:The expression of C-erbB-2,P53 and P21 in tumor areaswere studied in 90 breast carcinomas by immunohistochemical technique. Results: Positive rates of C-erbB-2, P53 and P21 in primary carcinomas were 64. 4%, 63. 3%. and 35. 6%, respectively. The rate ofpolygene overexpression was 57. 8%,positive rates of C-erbB-2,and P53 were higher than that of P21 (P〈0. 011 ). No significant difference was found between positive rates of the three genes in the metastat- ed lymphatic nodes and that in the primary tumor areas. The overexpressions of P53 and erbB-2 were ofpositive correlation with TNM, but not that of P21. Conclusion: The overexpression of C-erbB-2 or P53was associated with poor prognosis of breast cancer. Polygene overexpressions indicated the worse prog-nosis. Detecting the overexpression of C-erbB-2,P53 and P21 simultaneously in evaluating the prognosis ofbreast cancer was superior to doing alone.[
