脉搏波传导速度与冠状动脉病变和心血管事件的关系 被引量:11

摘要 目的:分析脉搏波传导速度(PWV)与冠状动脉病变和心血管事件的关系。方法:入选2005年10月至2007年9月在我院体检中心进行体检的否认既往心血管病史的人群,共入选319例,男性215例,女性104例,年龄(48.7±14.2)岁。根据脉搏波传导速度:分为脉搏波传导速度增快组(≥1 400 cm/s,1 480±79 cm/s)共95例,脉搏波传导速度正常组(<1 400 cm/s,1 030±350 cm/s)共224例。在6~18个月后进行电话随访。结果:脉搏波传导速度增快组行冠状动脉CTA检查,20例有不同程度的冠状动脉硬化和斑块(20/95,21.1%),正常组只有6例(6/224,0.27%),2组比较差异有统计学意义(P<0.05)。脉搏波传导速度增快组的颈动脉内-中膜厚度(0.23±0.08)cm高于正常组(0.14±0.05)cm,血压(152±23)mm Hg(1 mm Hg=0.133 KPa)高于正常组(114±26)mm Hg,2组比较差异有统计学意义(P<0.05)。脉搏波传导速度增快组的甘油三脂、总胆固醇、空腹血糖和正常组比较有升高趋势,但没有统计学意义(P>0.05)。电话随访6~18个月的心血管事件:脉搏波传导速度增快组发生心绞痛12例、入院治疗8例,正常组没有,比较差异有统计学意义(P<0.05)。结论:脉搏波传导速度增快的动脉硬化人群发生冠状动脉病变的风险增加,心血管事件的发生率增加。 Objective: To study the relation between pulse wave velocity and coronary atherosclerosis, cardiovascular events. Method:309 cases with physical examination were analyzed with computer software from 2006 to 2007,215 males and 104 females, (48.7 ± 14.2) yrs. All cases did not have cardiovascular disease. There were divided into two groups, pulse wave velocity increased group(PWV) ( ≥ 1 400 cm/s, 1 480 ± 79) cm/s, pulse wave velocity normal group (PWV) ( 〈 1 400 cm/s, 1 030 ± 350) cm/s. All cases were visited by phone from 6-18 month. Result: PWV ≥ 1 400 cm/s group was higher than the normal group in coronary arteries disease (21.1% vs 0.27 % ), there were significant difference( P 〈 0.05). PWV≥ 1 400 cm/s group was higher than the normal group in carotid arteries thickness (0.23 ± 0.08)cm vs (0.14 ± 0.05) cm and blood pressure(152 ± 23) mm Hg vs (114 ± 26) mm Hg). There were significant difference (P 〈 0.05). With the rise of the P WV, total triglycerides (TG), total cholesterol (TC), blood glucose showed increasing trends, but there are nut significant difference( P 〉 0.05 ). There was higher rate of cardiovascular events in PWV≥ 1 400 cm/s group than normal group. PWV≥ 1 400 cm/s group is higher than normal group in Angina pectoris and in hospital. Conclusion: With the rise of the PWV, coronary atherosclerosis, cardiovascular events increase.
