无创正压通气治疗Ⅰ型呼吸衰竭的疗效观察

摘要 目的探讨无创正压机械通气(NIPPV)在I型呼吸衰竭患者治疗中的应用价值。方法收集47例Ⅰ型呼吸衰竭患者的临床资料,所有患者均采用经口鼻面罩双水平正压通气(BiPAP)治疗,分析上机前后患者动脉血气、临床症状的变化和预后。结果47例患者无创正压机械通气治疗1h后,80.9%的患者临床症状及血气指标明显改善。28例使用后病情好转并成功撤机,有效率为59.6%;17例疗效不明显,其中6例改行气管插管,4例死亡;2例因不能耐受无创通气而放弃治疗。结论NIPPV可用于各种病因所致的Ⅰ型呼吸衰竭的治疗,有助于改善气体交换,降低插管率。
