摘要
目的:观察联合检测尿液四项微量蛋白质的临床意义。方法:采用全定量酶免疫测定法(EIA)分别检测126例肾病病人及30例正常人的尿微量白蛋白(micro-albumin,mAlb)、视黄醇结合蛋白(retinoi-binding protein,RBP)、β2-微球蛋白(β2-microglobulin,β2-MG)及免疫球蛋白G(immunoglobulin G,IgG)的含量,并将两者的检测结果进行比较。结果:①IgG:糖尿病性肾病组、高血压性肾病组和慢性肾炎组显著高于其他组(P<0.01);②mAlb:除高血压组和对照组无显著差异(P>0.05)外,其他观察组均明显高于对照组(P<0.01);③β2-MG:糖尿病组、糖尿病性肾病组和高血压性肾病组均明显高于对照组和其他观察组(P<0.01);④RBP:糖尿病组、糖尿病性肾病组和高血压性肾病组均明显高于对照组和其他观察组(P<0.01)。结论:mAlb、RBP、β2-MG可作为监测早期糖尿病肾病和高血压性肾病较敏感指标,尿微量蛋白联合检测对早期肾损伤有重要意义。
Objective: To determine the clinical significance of combined detection on four kinds of urine microproteins.Methods:The urinary micro-albumin(mAlb),retinoi-binding protein(RBP),β2-microglobulin and immunoglobulin G(IgG) in 126 patients and 30 normal people were detected by enzyme immunoassay(EIA) and comparisons were made between these groups.Results:① IgG: The levels of diabetes nephropathy group,hypertensive renal disease group and chronic nephritis group were significantly higher than that of other groups(P〈0.01).② mAlb: there was no significant difference between hypertension group and the control group(P〉0.05),but the levels of other four groups were higher than that of the control group(P〈0.01).③ β2-MG: The levels of diabetes group,diabetic nephropathy group and hypertensive renal disease group were higher than that of other groups(P〈0.01).④ RBP: The levels of diabetes group;diabetic nephropathy group and hypertensive renal disease group were higher than that of other groups(P〈0.01).Conclusion:RBP,mAlb and β2-MG may be used as sensitive indexes for diagnosing early diabetic nephropathy and hypertensive renal disease.Combined detection of urinary mAlb,RBP,β2-Mg and IgG is of great importance for early detection of renal damage.
