经腹腹腔镜肾癌根治术(附82例报告)

Transperitoneal laparoscopic radical nephrectomy(report of 82 cases)

目的探讨经腹途径腹腔镜肾癌根治切除术的技术要点及临床价值。方法肾癌82例,肿瘤平均直径5.5cm,左侧43例,右侧39例,69例肿瘤最大径≤7.0cm,13例肿瘤最大径>7.0cm,均采用经腹途径腹腔镜肾癌根治术。结果82例全部成功,无中转开放手术,手术时间70～230min,平均120min。术中及术后均未输血,无严重并发症。术后住院4～8d,平均6.1d。结论经腹途径腹腔镜肾癌根治术具有损伤小,安全有效,可行,恢复快等优点,有良好的临床应用前景。

Objective To present our experience with transperitoneal laparoscopic radical nephrectomy. Methods Eighty two cases of renal carcinoma were included in this study, 43 cases with left kidney lesion and 39 right kidney, mean diameter 5. 5cm. The maximum diameter of the tumors,size in 69 cases were not more than 7 cm and that in 13 cases beyond 7 cm. All patients underwent transperitoneal laparoscopic radical nephrectomy. Results All the operations in 82 cases of patients were successful. The operating time was 70-230 rain with a mean of 120 rain. No blood transfusion needed and no severe complications observed. The postoperative hospital stay was 4-8 d with average length of 6. 1 d. Conclusions Transperitoneal laparoscopic radical nephrectomy has advantages of minimal invasion, safety and effect, postoperative early resumption and shorter hospital stay,which has a good applied future in the urologic clinical practice.
