摘要
目的探讨还氧化酶-2(COX-2)蛋白在胃腺癌和胃黏膜不典型增生、正常胃黏膜中的表达情况,分析其表达与患者年龄、性别、肿瘤大小、大体形态、组织学分级以及淋巴结转移的关系。方法采用免疫组化ABC法,检测正常胃黏膜38例、非典型性增生胃黏膜36例及胃腺癌51例石蜡包埋标本中COX-2蛋白的表达,并分析其临床意义。结果COX-2蛋白在胃腺癌组织表达明显增强,其阳性率为64.71%(33/51),明显高于胃黏膜非典型增生的38.89%(14/36)及正常胃黏膜组织中的36.84%(14/38),差异有统计学意义(P<0.05);在胃腺癌中,淋巴结转移组阳性表达率为84.38%(27/32),淋巴结无转移组表达率为31.58%(6/19),差异有统计学意义(P<0.05);并且溃疡型胃癌阳性表达率为71.11%(32/45),非溃疡型胃癌阳性表达率为16.67%(1/6),差异有统计学意义(P<0.05);COX-2蛋白的表达与患者性别、年龄、肿瘤大小、组织学分级无相关性(P>0.05)。结论COX-2蛋白与胃癌的发生、发展及淋巴结转移有关。
Objective To investigate the expression of COX-2 protein in gastric adenocarcinoma and atypital hyperplasia and normal gastric mucosa,and to analyze the correlation between the expression of COX-2 and clinical pathological features such as patient s gender,age,general type,tumor s size,lymph node metastasis and histological grading.Methods ABC immunohistochemical method was used to detect the expression of PINCH in paraffin block from 38 cases of normal gastric mucosa tissue,36 cases of atypical hyperplasia and 51 cases of gastric adenoearcinoma. The relationship between the expression of COX-2 and the patients clinical pathological features was analyzed. Results The expression of COX-2 protein in tumor-associated stromal cells of gastric adenocarcinoma was heterogeneous and the positive rate was 64.71% (33/51), which was significantly higher than that (38.89%, 14/36) of atypical hyperplasia and that (36.84%, 14/38 ) in normal gastric mucosa (P 〈 0. 05) . The positive rate of the lymph node metastasis group(84.38% ,27/32) was significantly higher than that (31.58% ,6/19) of non-lymph node metastasis group. Furthermore the positive rate of the expression in ulcer gastric carcinoma was 71.11% (32/45), while which in non-ulcer gastric carcinoma was 16.67% ( 1/6 ), there was a significant difference between the two kinds of gastric carcinomas ( P 〈 0.05 ). But the expression COX-2 was not correlated with patients' gender, age, tumor size and histological grading ( P 〉 0.05 ). Conclusion COX-2 may play an important role in the genesis, development and lymph node metastasis of gastric carcinoma.
