血液肿瘤P-糖蛋白表达的初步研究

A Preliminary Study on Expression of P-glycoproteinin Hematological Malignancies

采用ＰＨＭＡＯ２单克隆抗体及细胞免疫化学法检测５４例血液肿瘤患者Ｐ－糖蛋白（Ｐ－ｇｌｙｃｏｐｒｏｔｅｉｎ，Ｐｇｐ）表达情况，以了解Ｐｇｐ这一多药耐药指标在临床上的意义。Ｐｇｐ表达与临床疗效的阳性符合率８９．４７％，阴性符合率９１．４３％，总符合率达９０．７４％。其中５０岁以上及反复化疗后的患者Ｐｇｐ阳性率显著增高（Ｐ＜０．０２；Ｐ＜０．００１）。慢性粒细胞白血病急变，骨髓增生异常综合征转化的白血病及急性非淋巴细胞性白血病Ｍ４亚型这几种类型血液肿瘤Ｐｇｐ出现高表达。提示Ｐｇｐ表达在判断血液肿瘤患者的疗效。

Expression of P-glycoprotein (Pgp) was determined using PHMAO_2 monoclonal antibody by immuno-cytochemical APAAP assay in 54 cases of hematological malignancies. The clinical significance of Pgp, a multidrug resistance index, was explored. The results showed that the positive, negative and general coincident rates between the Pgp expression and clinical curative effects were 89.47%, 91.43% and 90.74% respectively. Among these cases, the Pgp positive rates in patients of 50 and older, and in patients with repeated chemotherapy were significantly higher (P<0.02, P<0.01). The Pgp expression in these types of hematological malignancies, CML-BC, MDS-AL and ANLL-M4 tended to raise. Our results suggest that Pgp expression is important to determine curative effects and prognosis and to instruct treatment of the patients with hematological malignancies.
