热射病及经腹腔降温过程中深部体温的动态变化 被引量:2

DYNAMIC CHANGES OF CORE TEMPERATURE DURING HEAT STROKE AND BODY TEMPERATURE LOWERING THROUGH ABDOMINAL CAVITY

摘要 23只新西兰兔分为热射病组(n=11)、降温组(n=6)和对照组(n=6),研究热射病发生及经腹腔给予冷液降温过程中,直肠(Tr)、食道(Te)和下丘脑(Th)温度的动态变化。结果表明,Tr为42.5℃以前,Th高于Tr和Te;热射病休克发生时,Tr高于Te和Th。Tr、Te和Th随着温度的升高,上升速度减慢;高于41.5℃时,Te和Th上升速度显著减慢(P<0.05);高于42.5℃时,Th上升速度较前更慢(P<0.01)。与对照组相比,降温组动物存活时间显著延长(P<0.001),体温显著降低(P<0.001);在降温的开始20分钟内,Tr、Te和Th都急剧下降,Tr和Te出现明显降温高峰;第20分钟至动物死亡时,Tr和Te的下降速度比Th显著减慢(P<0.01)。提示:热射病及经腹腔降温过程中,深部体温的升降速度并不完全一致,腹腔冷液降温是热射病紧急情况下迅速降低体温的较好方法。 Study of dynamic changes of rectum(Tr), esopagus (Te) and hypothalamus (Th) during heatstroke and body temperature lowering through abdominal cavity by cold physiological saline wasconducted with 23 New Zealand rabbits divided into 3 groups: Group of heat stroke(n=11 ),temperature lowering (n= 6 ) and controls (n= 6 ). The results showed that when Tr was below42. 5℃,Th observed was higher than Tr and Te, however, went heat stroke occurred, Tr turnedhigher than Te and Th. With the rising of Tr, Te and Th,their increase rates decreased. When itwent up to >41. 5℃,the increasing rates of Te and Th were significantly decreased (P<0. 05).When >42. 5℃, the increasing rate of Th was much lower (P<0. 01). Compared with the controlgroup, the survival time of the temperature lowering group obviously prolonged (P<0. 001), andbody temperatures descended remarkably (P<0. 001). At the first 20 minutes of body temperaturelowering through abdominal cavity by cold saline, Tr,Te and Th all descended sharply, and thepesk descending value appeared in Tr and Te. From 20 minutes to death of rabbit, the descendingof Th was more remarkable than Tr and Te (P<0.01) did. It indicated that both ascending anddescending of the core body temperature were not completely the same during heat stroke and bodytemperature lowering through abdominal cavity by cold saline. This turns out to be a good way tolower the body temperature quickly in emergent case of heat stroke.
