两种方法治疗急性有机磷农药中毒疗效对比分析

Effect comparation between two ways to treat acute organophosphorus pesticide poisoning

目的对比分析阿托品配伍氯解磷定和盐酸戊乙奎醚配伍氯解磷定治疗急性有机磷农药中毒的效果。方法选择急性有机磷农药中度和重度中毒患者200例,按中毒至入院时间和病情分为两组,对照组100例使用阿托品配伍氯解磷定,观察组100例使用盐酸戊乙奎醚配伍氯解磷定,其它基础治疗方法两组相同。结果观察组治愈率99%,患者心率不增快,心肌缺血发生率3%,治疗第5天全血胆碱酯酶(ChE)可达正常值的50%以上,平均住院时间(7.6±1.1)天,平均医疗费用(3856.5±132.4)元;对照组治愈率92%,患者心率增快,心肌缺血发生率21%,治疗第8.5天ChE方达正常值的50%以上,平均住院时间(10.2±1.3)天,平均医疗费用(5102.5±106.5)元,两组各观察项目比较,差异均有显著性意义(P(0.01)。结论盐酸戊乙奎醚配伍氯解磷定治疗急性有机磷农药中毒起效快,效果好,毒副作用小,值得在基层医院推广。

Objective To compare the effects of atropine combined with pyraloximinmet hochloridum and long-acting Tuoning combined with pyraloximinmet hochloridum in the treatment of acute organophosphorus pesticide poisoning （ AOPP ）. Methods 200 cases of AOPP were divided into 2 groups, the control group was treated with atropine combined with pyraloximinmet hochloridum, and the test group was treated with long-acting Tuoning combined with pyraloximinmet hoehloridum. Results In test group, the cure rate of was 99% , no heart rate increase, the incidence of myocardial isehemia was 3% , ChE can achieve at least 50% of normal value in the fifth day of treatment,the mean length of stay in hospital was （7.6 ± 1.1 ） days. The mean medical expenses was （3856.5 ± 132. 4） yuan. Whereas in control group the cure rate was 92% , heart rate was increased, the incidence of myocardial ischemia was 21% , ChE can achieve 50% of normal value until 8.5 days of treatment,The mean length of stay in hospital was （ 10. 2± 1.3 ） days, the mean medical expenses was （5102. 5 ± 106. 5 ） yuan. Conclusions Long-acting Tuoning combined with pyraloximinmet hochloridum is better than atropine combined with pyraloximinmet hochloridum in the treatment of acute organophosphorus pesticide poisoning（AOPP）. It is worth to promote in the primary hospitals.
