MMP-9基因多态性与IgA肾病临床表现的关联性分析 被引量:10

Analysis of association between clinical manifestation and polymorphism of matrixmetalloproteinase-9 C-1562T gene in patients with IgA nephropathy

摘要 目的:研究中国长春地区汉族IgA肾病患者的MMP-9 C-1562T基因多态性与临床表现的关系。方法:应用聚合酶链反应-限制性片段长度多态性分析(PCR-RELP)技术检测85例IgA肾病患者的MMP-9C-1562T基因多态性,应用SPSS 13.0分析基因多态性与前驱感染、肉眼血尿、肾区疼痛、高血压、肾功能改变、是否表现为肾病综合征及尿蛋白定量等临床表现的关联性。结果:IgA肾病患者MMP-9 C-1562T基因型分布频数为:CC基因型62例(72.94%),CT基因型23例(27.06%),TT基因型缺如,符合Hardy-Weinberg平衡定律;CC和CT基因型与24 h尿蛋白定量有关联性(U=502.0,P=0.0454),与前驱感染、肉眼血尿、肾区疼痛、高血压、肾功能改变、是否表现为肾病综合征及镜下血尿程度等临床表现无关(P>0.05)。结论:临床上IgA肾病患者的MMP-9 C-1562T基因型可能影响尿蛋白程度,但不影响其他临床表现,亦不决定其预后。 Objective To study the relationship between matrixmetalloproteinase9(MMP-9) C-1562T gene polymorphism and the clinical manifestations in patients with IgA nephropathy.Methods The polymorphism of MMP-9 C-1562T gene in 85 IgA nephropathy patients were detected with polymerase chain reation-restriction fragment length polymorphism(PCR-RELP),SPSS13.0 was used to analyze the correlations between the gene polymorphism and pre-infection,gross hematuria,renal area pain,hypertension,renal fuction change,confirmed to clinical features of nephritic syndrome or not and the degree of microscopic hematuria.Results The genetypic frequency distributions of MMP-9 C-1562T gene in IgA nephropathy patients were CC 65(72.94%),CT 23(27.06%),TT 0(0%),they were not deviated from Hardy-Weinbery equilibrium.The genotypes of IgA may be associated with urinary protein(U=502.0,P=0.0454),unassociated with pre-infection,gross hematuria,renal area pain,hypertension,renal fuction change,confirmed to clinical features of nephritic syndrome or not and the degree of microscopic hematuria(P〉0.05).Conclusion On clinical,MMP-9 C-1562T genotype of IgA nephropathy patients may impact on the degree of urinary protein,but not other clinical manifestations,and prognosis of IgA nephropathy.
