大出血手术中血液回收机的应用及护理

Observing nursing of cell-saver in patients with massive hemorrhage during operation

目的总结血液回收机在手术中抢救大出血患者的应用及护理经验。方法21例术中大出血患者，采用负压吸引将手术创面的血液吸入储血罐内，经血液回收机处理将浓缩红细胞输给患者。结果21例患者术中反复出血3000～7800ml，平均每例回输失血约4500ml，术后回访，21例患者康复较好，未发生输血合并症，血液动力学稳定，肝肾功能和出凝血功能基本正常，伤口愈合良好，均痊愈出院。结论血液回收机在外科新型、高难度、大出血手术中的应用为患者接受外科治疗提供了安全保障，减少了异体输血，缓解了我国血源紧张、血液长期供不应求的基本现状，发挥了社会公益效益。

Objective To review and summarize the experience of application and medical care of using Cell-saver in 21 patients with massive bleeding during operation. Methods Aspiration of the operative blood from surgical wound into reservoir was performed and the blood was treated by Cell-saver and then re-transfused back to patients. Results Average 4500 ml blood was re-transfused back to 21 patients with intra-operative bleeding of 3000 -7800 ml, Follow-up data show that all patients in the study recovered satisfactarily. No adverse events happened and haemodynamics was stable. Biochemistry parameters were normal and incision healing was satisfactory. Conclusions Cell-saver has ensured the patients safety during complicated surgical procedures and cut down the requirement of blood transfusion, alleviated the level of blood shortage in the country.
