摘要
目的:探讨2型糖尿病(T2DM)、高血压病(EH)及二者并发对血管内皮功能的影响及与之相关的因素,为防治T2DM合并EH患者内皮损伤提供新的依据及治疗指导。方法:采用酶联免疫吸附双抗体夹心法测定T2DM、EH、T2DM+EH患者血浆内皮损伤标志物血管性血友病因子(vWF)百分含量(vWF%),并与对照(NC)组进行比较。结果:vWF%T2DM组(115.81±38.56)%、EH组(112.97±32.47)%、T2DM+EH组(133.77±42.75)%均高于NC组(92.29±23.97)%;T2DM+EH组分别高于T2DM组、EH组,差异有统计学意义(P<0.05或P<0.01)。NC组BMI,T2DM组FIN、HbA1c,EH组收缩压(SBP),T2DM+EH组SBP、HbA1c进入影响血浆vWF%独立相关因素多元逐步回归分析方程。结论:T2DM、EH患者存在内皮功能损伤。NC组BMI;T2DM组FIN、HbA1c;EH组SBP;T2DM+EH组SBP、HbA1c是影响血浆vWF%的独立相关因素。
Objective: To investigate the effects of type 2 diabetes mellitus (T2DM)and essential hypertension (EH)on endothelium function and the correlative factors, and to provide new evidence and management guideline for preventing and curing the impaired endothelium in patients with T2DM complicated with EH. Methods: The plasma yon Willebrand factor (vWF) percentage content (vWF%) was measured by enzyme linked immunosorbent assay(ELISA), in the following four groups: control group (NC), T2DM group, EH group and T2DM+EH group. Results: The values of vWF% were higher in group T2DM (115.81 ± 38.56)%, EH(112.97±32.47)% and T2DM+EH (133.77±42.75)% than that in NC(92.29± 23.97 )% group(P 〈 0.05). The values of vWF % were higher in group T2DM+EH than those in T2DM and EH groups(P 〈 0.05). The body mass index(BMI) of group NC, fasting insulin(FIN) and HbA1c of group T2DM, systolic blood pressure(SBP) of group EH, SBP and HbA1C of group T2DM +EH went into muhivariant stepwise regression analysis equations that were independent correlative factors which affected plasma vWF%. Conclusion: There was endothelium dysfunction in T2DM and EH patients. The endothelium dysfunction was found the most severe in T2DM+EH patients of the all groups. BMI of group NC, FIN and HbA1c of group T2DM, SBP of group EH, SBP and HbA1c of group T2DM+EH were independent correlative factor that affected plasma vWF %.
