增强型体外反搏对突发性聋患者血清一氧化氮、丙二醛和超氧化物歧化酶的影响

摘要 目的观察增强型体外反搏(EECP)对突发性聋患者的临床疗效及对血清一氧化氮(NO)、丙二醛(MDA)和超氧化物歧化酶(SOD)的影响,探讨EECP治疗突发性聋的机制。方法将220例突发性聋患者随机分成体外反搏组(120例)和单纯药物组(100例),治疗前后做纯音测听判断疗效。同时收集50例突发性聋患者(体外反搏组30例,单纯药物组20例)治疗前后和20例健康对照者外周静脉血清,采用硝酸盐还原酶法测定各血清中NO3-和NO2-的含量(反映NO的浓度),用比色法测定MDA和SOD的含量,并进行各组分析比较及NO、MDA和SOD相关性分析。结果体外反搏组总有效率(86.67%)优于药物组(74.00%),差异有统计学意义(P<0.05)。结论体外反搏是突发性聋的一种安全有效的辅助治疗方法。突发性聋患者呈血管内皮功能失调,脂质过氧化反应增强状态。增强型体外反搏可促进NO的释放,从而调整内皮功能,并降低MDA的产生,促进SOD的释放,减轻脂质过氧化反应,这可能是增强型体外反搏在临床上治疗突发性聋的机制之一。 Objective To evaluate the clinical efficacy and to investigate the effect of enhanced external counterpulsation (EECP) on serum nitric oxide (NO) ,malondialdehyde (MDA) and superoxide dismutase (SOD) in the patients with sudden deafness (SD) and its mechanism. Methods 220 patients with SD were divided into EECP group (120 cases) and medication group (100 cases). The improvement of auditus was observed. The serum concentrations of NO,MDA and SOD of 50 cases (30 of EECP group, 20 of medication group) ,and 20 normal controls were detected before and after treatment. Results For total patients, the effective power of auditus was higher in EECP group (86.67 %) than in medication group (74.00 % ). Conclusion EECP can treat SD effectively and safely. The blood vessel endothelium function is disordered and lipid peroxidation is enhanced in SD patients. EECP treatment can regulate blood vessel endothelium function by improving the release of NO and relieve lipid peroxidation by reducing the production of MDA or increasing the production of SOD. The results may be useful in further study of EECP mechanism.
