摘要
目的:分析抑郁症患者睡眠多导图(PSG)表现。方法:对45例抑郁症患者治疗前后进行PSG和汉密尔顿抑郁量表(HAMD)测定。以43例健康人为对照。结果:抑郁症组治疗前与对照组相比,PSG表现为总睡眠时间(TST)缩短、睡眠潜伏期(SL)延长、觉醒次数(AT)增加、REM潜伏期(RL)及REM时间(RT)缩短、REM密度(RD)增加,差异均有统计学意义(t分别为58.12,17.19,22.32,15.00,5.60和4.95,P<0.05);治疗后上述指标与对照组比较,差异无统计学意义(t分别为2.13,1.35,2.47,3.39,2.30,0.38,P>0.05)。抑郁症组治疗后HAMD-17评分(7.86±2.37)较治疗前的(28.12±10.14)降低(t=12.33,P<0.01)。抑郁症患者治疗前PSG测定中的TST、S3+S4、RL和RT与HAMD-17分值成负相关(r分别为-0.34、-0.31、-0.39和-0.37,P均<0.05),SL、AT及RD与HAMD-17分值成正相关(r分别为0.30、0.38和0.42,P均<0.05)。结论:抑郁症患者的PSG有特异性表现,对抑郁症患者的诊治有较好的应用价值。
Aim: To analyze the polysomnogram (PSG)of depression patients. Methods:Forty-five depression patients were tested with PSG and HAMD before and after treatment,and forty-three healthy people were the control. Results: Compared with those of the control group, in depression group before treatment, the total sleep time(TST) decreased, sleep latency(SL) prolonged, awareness frequency(AT) increased, REM latency and REM time decreased, and REM density increased ( t was 58.12,17.19,22.32,15.00,5.60 and 4.95, respectively,P 〈 0.05 ) ; the indexes mentioned above of depression group after treatment have no differences from those of the control( t was 2.13,1.35,2.47,3.39,2.30 and 0.38, respectively,P 〉 0.05 ). The scores of HAMD-17 in depression group after treatment were (7.86 ± 2.37) , significant lower than (28.12 ± 10.14) before treatment(t = 12.33 ,P 〈 0. 01 ). TST, deep sleep time( S3 ±S4), REM latency and REM time of PSG in depression group before treatment were remarkable negatively correlated with the scores of HAMD-17 ( r was -0.34, -0.31, -0.39 and -0.37,respectively,P 〈 0. 01 ) ,while SL, AT and REM density were positively correlated with the scores of HAMD-17 ( r = 0.30,0.38 and 0.42, respectively, P 〈 0. 01 ). Conclusion : Depression patients have obvious sleep impairment, PSG can reflect sleep status and has remarkable clinical value for approval sleep of depression depression.
