可吸收螺钉治疗下胫腓联合损伤21例分析

The use of absorbable screws in tibiofibular syndesmosis injury treatment of 21 cases

目的讨论可吸收螺钉(PDLLA)选择性治疗下胫腓联合损伤的临床疗效。方法2003年9月至2008年7月,作者采用可吸收螺钉选择性内固定治疗21例踝关节骨折中合并下胫腓联合损伤。AO-Danis-Weber分型:B3型5例,C1型10例,C2型6例。结果患者伤口均Ⅰ期愈合,无皮缘坏死,内固定无外露无移位;均无感染,无非特异性积液等并发症发生。21例患者均获随访,随访时间3～12个月,平均9.3月。术后21例骨折均临床愈合,愈合时间12～18周。术后患者踝关节背伸20～30°,跖屈40～50°。疗效评定按照Baird Jackson踝关节功能评分标准本组优16例,良3例,可1例,优良率为97.1%。结论PDLLA螺钉是治疗下胫腓联合损伤的一种良好方法。

Objective To discussing the clinical efficacy of self reinforced absorbable screws for Tibiofibular syndesmosis injury treatment. Methods The authors had treated 21 cases of Tihiofibular syndesmosis injury using absorbable screws Weber B3 five cases, weber C1 ll cases and weber C2 six cases. All of the cases had been followed up. The time is three to 12 month （about 9.3 months）. Results The wound of all patients healed in grade Ⅰ. No wound infection and fluid accumulation occured. All the cases were followed up for three to twelve months （average 9.3 months）. According to the functional criteria, 16 patients achieved excellent, three achieved well and one achieved common. The excellent and good rate is 97.1%. Conclusion The authors believe that the absorbable screw are ideal implants for the treatment of Tibiofibular syndesmosis injury.
