摘要
目的比较呼吸道合胞病毒(RSV)肺炎患儿与非RSV肺炎患儿血清胆红素水平,探讨因RSV感染导致肺炎的新生儿总胆红素较低的原因。方法对2007年9月到2008年5月期间我院因肺炎住院新生儿399例进行回顾性分析。应用直接免疫荧光法对急性呼吸道感染患儿鼻咽分泌物进行RSV等7种病毒抗原检测。以RSV抗原阴性及阳性结果不同分为两组,对两组患儿总胆红素(TBIL),直接胆红素(DBIL),丙氨酸氨基转移酶(ALT),γ谷氨酰转肽酶(γ-GT),血清前白蛋白(PAB)进行分析。结果所检测399例标本中RSV阳性149例,RSV阴性者250例。RSV肺炎患儿PAB以及TB IL水平较其他类型肺炎患儿低,两组中γ-GT、DB IL、ALT水平均无显著差异。结论新生儿RSV感染后胆红素的代谢过程可能有改变。
Objective To compare TBIL level in neonatal respiratory syncytial virus (RSV) pneumonia with that in healthy neonates and analyze the possible mechanism of the decreased TBIL in respiratory syncytial virus(RSV) pneumonia. Methods A retrospective investigation was conducted in 399 neonates with pneumonia hospitalized( from September 2007 to May 2008 ) in Chirldren's Hospital, Soochow University. Nasopharyngeal aspirates were obtained from the respiratory tract by aseptic vacuum aspiration. Direct immuno - fluorescence assay was employed to detect seven kinds of virus antigens including RSV antigen. All neonates were divided into 2 groups in according to the result of the test. TBIL, DBIL, ALT, γ- GT, PAB were analyzed. Results RSV positive cases were 149. Compared with healthy neonates, neonates with RSV pneumonia showed lower levels of TBIL and PAB, but similar levels of γ - GT, DBIL and ALT. Conclusion An alteration of bilirubinic metabolic process may occur when a neonate infects RSV.
