摘要
目的探讨应用速度向量成像(VVI)技术检测2型糖尿病患者左心室心肌舒张功能的应用价值。方法对44例2型糖尿病患者根据临床糖尿病微血管病变出现与否分为无微血管病变组(A组)21例,合并微血管病变组(B组)23例;另选体检正常对照组22例,采用VVI技术测量左心室短轴方向各壁的舒张早期最大速度(Ve)与舒张早期最大应变率(SRe)。结果A组Ve仅5个节段(基底后壁,中部前壁、侧壁和后壁,心尖侧壁)较对照组显著降低(P<0.05);B组大部分节段(中部后壁、下壁和后间隔,心尖部室间隔除外)较对照组显著降低(P<0.05)。A组的多数节段SRe较对照组显著降低(基底及中部侧壁、中部前间隔除外),B组多数节段SRe均较对照组显著降低(中部前间隔除外);A、B组之间比较,仅基底部侧壁及后间隔有差异(P<0.05)。结论VVI技术可用于评价2型糖尿病患者心肌舒张功能,VVI参数SRe有望成为一种评价糖尿病心肌病变的新指标。
Objective To study the velocity and strain rate of cardiac diastolic motion by using veloci- ty vector imaging (VVI) in the type 2 diabetes mellitus patients. Methods A total of 44 patients with type 2 diabetes mellitus were classified into two groups: Group A included 21 patients without microangiopathy and Group B included 23 patients with microangiopathy and 22 objects were recruited as the healthy control group examined with VVI. The maximum velocity in early diastolic phase (Ve) and the maximum strain rate in early diastolic phase (SRe) of the left ventricular walls were measured. Results The Ve was lower in A group than that in the control group for five segments. In the group B, the Ve was lower in 12 segmems(P 〈0.05). The SRe was lower in group A than in the control groups with respect to 13 segments. Except middle segment of interventricular septum, the SRe was lower in the group B than in the control groups for every segments( P 〈 0.05 ). Conclusions The VVI can evaluate the diastolic function objectively in patients with type 2 diabetes mellitus and the SRe can become a useful index in assessing the diabetic cardiomyopathy.
